<i>a sin(kx)</i> has amplitude a and period 2pi/k, so you can use your requirements to produce the function

I just took the periodic Functions and Trig Unit Test Part 1 and relied on cleepus's answers and literally got 7/24 so just google the questions individually, you will prolly get better results.
