=
266 / 4913(x-4)
- 266(x+4) / (x^2+1)
+ 23(x+4) / 289(x^2+1)^2
+ (7-11x) / 17(x^2+1)^3

and there is no (x-4)^2 term, since your original fraction had none. If you really want that to be included, I can only refer you to
