I assume you mean

y = -2√(3x-12)-5
= -2√(3(x-4)) - 5

so, the function √x has been shifted right by 4, scaled horizontally by a factor of 1/3, reflected over the x-axis, stretched vertically by a factor of 2, and shifted down 5.
