#1 is correct IF there are other Toms and you're referring to the one "who lives there."
#2 is preferable because usually, when you use someone's name, you are referring only to that one person.
#3 and #4 are correct. Either way works fine.
1. I love Tom who lives there.
2. I love Tom, who lives there.
